How does Terbinafine work?
Terbinafine is an antifungal medication. The active ingredient, terbinafine hydrochloride, is an enzyme inhibitor. It prevents the action of an enzyme which leads to fungal cells being unable to remove certain toxins, and to rebuild their cell walls.
Together, this means fungal cells will die off and be unable to reproduce when exposed to this medication. Your immune system can remove the rest of the fungal cells.
The treatment course for this medication is between six weeks and 3 months. In some cases, treatment can take as long as six months. This ensures all the fungal cells have been removed, and will not return after treatment.
While many patients use terbinafine for nail fungus, it can be used for a variety of fungal skin infections. This includes athlete's foot, jock itch (tinea cruris) and infections in other body parts. We do not supply terbinafine for ringworm or other skin conditions.
This medication is also sold under the brand name Lamisil.
How to tell if Terbinafine is working
Due to the slow growth of nails, it may take time to see results. Generally, your nail may still look infected, but you should start to see new healthy nail grow out from the nail bed. As your nail grows, the infected part will grow out until you are left with healthy nails. You can clip off the infected part as it grows out.
How to take Terbinafine 250 mg tablets
Take one tablet each day, at roughly the same time. Take with water.
If you miss a dose, take your next dose as soon as possible when you remember. If it is the next day, skip the missed dose - do not take an extra tablet to “catch up”.
THe duration of treatment will depend on where the infection is located;
- Fingernail- minimum of 6 weeks treatment needed
- Toenails- minimum of 3 months treatment needed.
Treatment may be required for longer depending on the severity of the infection.
Terbinafine Side Effects
As with all medications, terbinafine tablets may cause side effects in some patients.
The most common side effects of terbinafine may include;
- stomach ache and loss of appetite
- diarrhoea
- indigestion and nausea
- joint and muscle pain
- skin reactions, including hives and itching
- headache
These symptoms will affect around 1 in 10 people who use this medication.
Stop taking this medication and seek medical attention if you experience any of the following effects;
- swelling of the face or throat
- difficulty breathing or dizziness
- muscle stiffness
- rashes and fever
- swelling of the lymph nodes
These may be signs of a serious allergic reaction.
Stop treatment if you have symptoms of;
- unexplained persistent nausea
- decreased appetite
- fatigue
- vomiting
- right upper abdominal pain
- jaundice
- dark urine or pale faeces
These signs could indicate liver damage. Stop treatment and seek immediate medication attention if you experience any of these symptoms.
We also recommend that patients should undergo a liver function test 4-6 weeks after starting treatment (but this is not essential).

Warnings
Do not take Terbinafine if;
- you are allergic to terbinafine, or any other listed ingredients of this medication
- you have a severe problem with your kidneys or liver
- you are breastfeeding
Speak to your doctor or pharmacist before using this treatment if;
- you have any existing kidney or liver problems
- you have skin conditions including psoriasis
Stop using this treatment and seek urgent medical attention if you;
- develop a reduction in white blood cells or blistering of the skin
- experience any other severe side effects
Talk to your doctor or pharmacist if you are using or have recently used any other medications. In particular, if you are using;
- rifampicin
- cimetidine
- antidepressants
- anti-arrhythmics or beta-blockers
- oral contraception (the pill)
- propafenone, amiodarone or any other medicines to treat heart problems
- ciclosporin
- tolbutamide
- triazolam
- terfenadine
- fluconazole, ketoconazole or other medicines to treat fungal infections
- dextromethorphan or other cough medicines
- caffeine
Always store out of reach of children and pets.
Terbinafine and pregnancy
This treatment is not suitable for pregnant people or those who are breastfeeding without their doctor’s explicit approval. You should let your doctor know as soon as possible if you become pregnant while taking this medication.

FAQ
How long does terbinafine take to work?
Treatment with terbinafine tablets normally take between 6 to 12 weeks to fully clear a fungal nail infection. In some cases, treatment can last up to 6 months to fully clear an infection and prevent the infection coming back.
Can I drink alcohol while taking anti-fungal medication?
The patient information has no warnings against drinking alcohol with terbinafine. However, you should avoid caffeine while using this treatment.
Can I use terbinafine cream instead of terbinafine pills?
Terbinafine cream is not suitable for treating a fungal nail infection. Currently, we only supply tablets.
What should I do if I miss a dose of terbinafine?
If you miss a scheduled dose, take it as soon as possible when you remember. If it is the next day, do not take an extra dose to “catch up”, simply skip the missed dose.
Can I use terbinafine if I am pregnant?
You should not use terbinafine or other antifungal treatments while pregnant or breastfeeding.
Does terbinafine affect fertility?
There are no current concerns about fertility while using this treatment.
Can I take fluconazole and terbinafine together?
No. Mixing antifungal medications is not recommended, and fluconazole can increase the levels of terbinafine in your body. This can lead to more severe side effects.
When is the best time to take terbinafine 250 mg?
You can take this treatment at any time that suits you. Many patients may choose to take it in the morning as part of their routine, to avoid forgetting to take it.
